CASE MANAGER

SUMMARY:
This person is responsible for overseeing a caseload of foster parents and foster youth in The Unique Caring Foundation, Inc. while assisting with maintaining the requirements for all licensure and national accreditation. The responsibilities as outlined below:

JOB RESPONSIBILITY:
  • Responsible for overseeing the authorization, re-authorization, and discharges of the foster youth on the assigned caseload.
  • Maintain current authorizations on ALL individuals on the caseload at 100% accuracy.
  • Submit re-authorizations to MCO within the 10-14 suggested dates PRIOR to the expiration of authorizations with 100% accuracy.
  • Attend treatment team meetings weekly for therapeutic youth and monthly for all DSS family youth on caseload making PCP recommendations as needed.
  • Facilitate monthly treatment meetings for assigned caseload and update of PCP, within 72 hours of the treatment meeting, ensuring the plans are person-centered.
  • Complete the Discharge Summary form upon the separation of each foster youth on the assigned caseload.
  • Monitor each foster home to ensure the environment remains safe according to licensure standards.
  • Monitor each therapeutic home to ensure the foster youth is receiving therapy as recommended by the assigned therapist, with 100% compliance.
  • Completion of incident and accident reports according to state mandates and the timely submission, with a minimum of 97% accuracy.
  • Ensure that the state-mandated therapeutic home visits are a minimum of weekly face-to-face visits or as stated in the PCP, to maintain optimum quality of service delivery.
  • Comply with company policy by making telephone contact with each foster home at least once a week to ensure that their needs are met with a minimum of 100% compliance.
  • Ensure that all case management paperwork and documentation are submitted to the UCF department in a timely manner with a 98% compliance rate.
  • Ensure foster parents are providing the services in compliance with the PCP daily.
  • First responder to any questions or problems with the foster parents and youths on your caseload.
  • Maintain optimum communication with staff, stakeholders, and referring agencies ensuring that all information needed for effective job performance is received.
  • Attend pertinent training offered by this agency, contracted agency, MCO, or MCO affiliate to ensure that you are equipped with the latest information needed to perform your job at optimum.
  • Attend state-required 24 hours of pertinent training annually.
  • Observe and maintain confidentiality as mandated in HIPAA regulations and that of UCF.
  • Maintain all Client Rights and ensure there is no abuse or neglect taking place in the foster home.
  • Responds appropriately to all inquiries following all company policies, procedures rules, and regulations.
  • Perform other duties as assigned.

REQUIREMENTS:
  • Must meet at a minimum the qualifications for QP status as stipulated by the state.
  • Must have a 4-year Bachelor's degree in the Human Services field.
  • Must have at least 2 years of job-related experience.
  • Ability to analyze and problem solve.
  • Ability to work as a team member.
  • Ability to multitask.
  • Excellent organization skills.
  • Must demonstrate excellent interpersonal and communication skills.
  • Excellent written and oral communication skills.
  • Excellent ethical work ethics.
  • Prefer certification in First Aid, CPR, EBPI, Blood-borne Pathogens, Seizure Management, and HIPAA.
  • Prefer training in areas of Child Development, Permanency Planning Methodology, Family Systems and Relationships, Child Sexual Abuse, Trauma-Informed Care, and the Reasonable and Prudent Parent Standard as required by the state.

ADDITIONAL REQUIREMENTS:
  • Prefer Bachelor in Social Work or Health and Human Services field.
  • Computer experience with Word, Excel, and other computer software.
